Application Information
We are currently accepting applications for enrollment for 2008-2009 school year. Please fill out the application form provided at the link below. We will then contact you for enrollment on a “first come first serve” basis.
If you would like to apply for the current school year, please contact our secretary Mrs. Melanie Knight at mknight@starsmiddleschool.org or at 681-7827 ext:601 for available openings.

The School Tours for prospective students and parents are held every Thursday at 3:15 p.m. Prospective parents should call 681-7827 or e-mail mknight@starsmiddleschool.org to schedule.

Parents:

School opens at 7:00 am and the students can not be dropped off before this time.

Students need to be dropped off to cafeteria between 7:00 and 7:32 am. They will be dismissed to their classes from cafeteria at 7:32am.

If you are arriving after 7:40 am in the morning, please see the secretary at the front office to sign your child in.

HEALTH DEPARTMENT PROVIDES SPECIAL IMMUNIZATION CLINICS FOR CHILDREN NEEDING BACK-TO-SCHOOL SHOTS

Students who will be entering kindergarten or 7th grade next school year must have certain immunization documentation on file at school before the start of the new school year. Students entering kindergarten need documentation of 5 DTaP, 4 polio, 2 measles, 2 mumps. 2 rubella, 3 Hepatitis B, and 2 varicella (chickenpox) vaccines. Students entering 7th grade need documentation of a Td/Tdap booster vaccine, which is typically given at 11 -12 years of age. Parents are urged to have their children immunized now for the 2008 - 2009 school year!

The Leon County Health Department provides routine back-to-school immunizations Monday through Friday on an appointment basis. In addition, the Health Department will provide special extended hours immunization clinics targeting students needing immunizations for school attendance. Infants and young children needing immunizations may also be immunized at these special clinics. While an appointment is encouraged, children may be seen on a walk-in, space available basis during the free immunization clinics listed below:

All children receiving immunizations through the Health Department must be accompanied by a parent or legal guardian. Parents should bring a copy of their child's complete immunization record.
